Home Health Care Navigator

Otterbein SeniorLife is a nonprofit senior living and care organization with more than 100 years of service; we are seeking a Home Health Care Navigator to drive post-acute skilled home health referrals and census growth across the greater Dayton/Lebanon/Cincinnati, OH territory. The role focuses on building and maintaining relationships with hospitals, skilled nursing facilities, physicians, discharge planners and community referral sources to ensure smooth transitions to Otterbein Home Health and meet financial and census goals.

Skills

  • Proven relationship-building and referral marketing skills
  • Strong verbal and written communication for liaising with clinicians and referral partners
  • Organizational, planning and time-management capabilities
  • Comfort working independently in a territory-based, community-facing role
  • Ability to make timely, accurate decisions related to patient admissions and transitions

Responsibilities

  • Develop and maintain relationships with administrators, physicians, social workers and discharge planners to drive referrals for skilled home health services.
  • Assess referral potential, gather required orders/documentation, and coordinate physician signatures to admit patients to Otterbein Home Health.
  • Monitor and maintain targeted census levels to meet financial and operational goals.
  • Implement and support local marketing plans, distribute promotional materials, and participate in community outreach (Chambers, senior centers, churches, etc.).
  • Coordinate with Otterbein team members to ensure smooth patient transitions and communicate anticipated referrals/admissions to staff.
  • Identify process improvements and opportunities to simplify referral pathways for community partners.
  • Track referral sources and maintain CRM/data records to support targeted marketing and reporting.

Education

  • BA/BS preferred
  • LPN/RN or social work background preferred (for clinical liaison experience)
