Responsibilities
- Manage a primary hospice caseload and provide skilled nursing care in accordance with physician orders.
- Perform comprehensive nursing assessments and develop/update individualized hospice care plans.
- Provide symptom management, medication administration, and clinical interventions for end-of-life care.
- Educate and support patients, families, and caregivers about disease progression, care needs, and comfort measures.
- Coordinate care and communicate regularly with interdisciplinary team members and community resources.
- Document timely, accurate clinical notes and reports in compliance with Medicare Hospice Conditions of Participation, Ohio licensure laws, and agency policies.
- Participate in case conferences, quality assurance activities, and regulatory compliance efforts.
- Maintain professional licensure, complete required training, and follow agency policies and procedures.
Education
- Graduate of an approved nursing program (ADN or BSN)
- Bachelor of Science in Nursing (BSN) preferred
- Valid Ohio Registered Nurse (RN) license
- Current CPR/BLS certification (or willingness to obtain)
