Hospice Registered Nurse RN - Case Manager

Otterbein SeniorLife is a faith-based, not-for-profit leader in senior living and hospice services seeking a compassionate Hospice RN Case Manager to support the growth of its Hospice ministry in Toledo. The RN Case Manager will manage a primary caseload, deliver skilled nursing care consistent with physician orders and hospice regulations, coordinate with interdisciplinary teams, and provide patient- and family-centered end-of-life support.

  • Current Ohio Registered Nurse (RN) license
  • Skilled patient assessment and clinical decision-making
  • Hospice and end-of-life symptom management
  • Medication administration and wound care skills
  • Clear, compassionate communication with patients and families
  • Care planning and case coordination
  • Accurate clinical documentation and regulatory compliance
  • Familiarity with electronic medical records (EMR/EHR) systems

Responsibilities

  • Manage a primary hospice caseload and provide skilled nursing care in accordance with physician orders.
  • Perform comprehensive nursing assessments and develop/update individualized hospice care plans.
  • Provide symptom management, medication administration, and clinical interventions for end-of-life care.
  • Educate and support patients, families, and caregivers about disease progression, care needs, and comfort measures.
  • Coordinate care and communicate regularly with interdisciplinary team members and community resources.
  • Document timely, accurate clinical notes and reports in compliance with Medicare Hospice Conditions of Participation, Ohio licensure laws, and agency policies.
  • Participate in case conferences, quality assurance activities, and regulatory compliance efforts.
  • Maintain professional licensure, complete required training, and follow agency policies and procedures.

Education

  • Graduate of an approved nursing program (ADN or BSN)
  • Bachelor of Science in Nursing (BSN) preferred
  • Valid Ohio Registered Nurse (RN) license
  • Current CPR/BLS certification (or willingness to obtain)
