Job Title: Infusion & Specialty Pharmacy Nurse Case Manager
Department: Operations

General Functions
Provides designated or dedicated services for a Pharmaceutical Manufacturer or Program. Serves as a point of contact to build enduring relationships between the Company, Pharmaceutical Manufacturer and other Program Partners connected with contracted services. Collaborates with providers, nurses and other healthcare professionals to prepare patients and families for their care journey. This individual monitors all aspects of the program to ensure that services are compliant with terms of partner agreements, meeting requirements for performance and deliverables.

Primary Responsibilities:
Assumes designated or dedicated contract, product or program duties aligned with position.
Participates in program development and implementation of new programs. Ensures internal teams are aware of new program requirements. Interprets and disseminates key program/product information.
Provides education/training when modifications are made to the existing program with respect to contract amendments including key information about updates or changes in the assigned product and/or program details.
Acts as a liaison between Company, Pharma Manufacturer and Pharma Manufacturer partners (e.g., HUB, data aggregator, etc.).
Ensure operational and clinical aspects of Specialty Pharmacy’s Services and Core SP Activities are meeting contractual requirements, including working with both Specialty Pharmacy’s clinical and non-clinical teams who are involved in the performance of Services and Core SP Activities.
Work closely with the Program Liaison (described below) to identify Patient Product access challenges and to efficiently and appropriately address the Product access challenges with the appropriate internal and/or external stakeholders.
Operate pharmacy software systems to accurately input nursing information and obtain necessary Patient information to work effectively with internal and external parties in relation to patient care.
Serve as the point of contact regarding implementation and day-to-day operational issues, both internally and with Manufacturer’s Specialty Pharmacy point of contract.
Conduct conference calls or meetings as needed with Manufacturer personnel to review Service performance.
Conduct conference calls or meetings, as needed with appropriate third parties, including Hub personnel, home health nursing agencies, and provider office/clinic staff to facilitate Patient access to Product and communicate to Manufacturer information received from such third parties regarding potential process improvements.
Provide appropriate information about Product to providers/clinic staff, including addressing and providing information on Product storage and administration. Refer external parties to Manufacturer clinical support when needed.
Identify operational or clinical initiatives and modifications to current process to improve the Patient and provider experience.
Participate in scheduled business reviews as requested by Manufacturer, to discuss performance of Services under this Agreement.
Communicate with Manufacturer regarding Product marketplace conditions, provider concerns and experiences, home nursing reports/experiences, payer coverage issues, etc.
Participates in meetings with the Manufacturer HUB, home health agencies, and provider offices/clinic staff to assist our core staff in facilitating access to Product.
Serves as a clinical resource for questions on infusion or Program Product use, dosing, storage and administration. Refers internal and external parties to Manufacturer support when needed.
Assists with hospital to home transition, infusion suite and other program development from a nursing perspective.
Provides direct patient care in person or telephonically.
Adheres to all Company policies as indicated in the handbook and directives issued by management. Has reviewed Policy and Procedure manual.
Responsible to assist with Pharma Program audits and participate in on-site or virtual audits including but not limited to planning and preparation of documents, notification to appropriate team members, proper follow-up documentation required for partner and management of appropriate resolutions, documentation training and implementation of action items from results.

Highest business acumen, credibility with colleagues, and communication skills required to represent requirements to cross-functional teams and partner with Manufacturers or other stakeholders.
Required Qualifications:
At least 3 years of prior nursing experience with strong infusion skills in hospital, home infusion or IV team setting.
Prior teaching and training experience on infused drug therapies, home or office-based infusion procedures for assigned product line(s), and experience with appropriate discharge planning/coordination preferred.
Healthcare management background preferred.
Registered/licensed nurse within state-specific guidelines required.
Strong knowledge of the infusion and specialty pharmacy industry, daily operational workflow, specialty medications, and chronic, complex disease states they are used in is preferred.
Ability to travel if needed – minimal travel is expected with this position.
Educational Requirements:
Bachelor’s degree in Nursing
Licensure and Certification Requirements:
Current unrestricted state registration/licensure (if applicable). Maintains continuing educational requirements to keep certification active. Additional licensure may be required in multi-state service areas.
Physical Requirements:
Must be able to remain in a stationary position up to 90% of the time.
The person in this position needs to occasionally move about inside the office to access file cabinets, office machinery, etc. and traverse conferences, meetings, and remote events.
Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
The ability to observe details at close range (within a few feet of the observer).

Computers, keyboards, mouse, monitors, fax, and/or headsets for phone work. Software specific to the position, including but not limited to Microsoft Outlook and Skype. Must lift and traverse the area to move paper and supplies to use the equipment.
Contacts:
Frequently interact with co-workers, intradepartmental staff of the Company, managers, leaders, Vendors, and customers. High interactions with physicians and the medical community to support the sales and perfection of the referral to fill.
