Responsibilities
- Serve as initial point of contact for referred patients and facilitate enrollment through intake completion
- Communicate with patients via text, phone, and video to drive enrollment, engagement, reminders, and follow-up
- Send mental health screenings, track responses, and follow up on results
- Update patient records in clinic EHRs and internal FamilyWell EHR; maintain accurate documentation
- Maintain and update the collaborative care registry to track follow-up and clinical outcomes
- Prepare and submit monthly patient progress reports in the EHR
- Escalate clinical concerns and updates to the Care Manager in a timely manner
- Compose and communicate timely patient and provider responses; coordinate with clinic staff
Education
- Bachelor’s degree in nursing, social work, psychology OR formalized training in mental health
- Clinical licensure not required for this role
