The Children’s Treatment Family Care Recruiter and Case Manager position has dual responsibilities within two mental health programs. First, the position is responsible for the recruitment, training and retention of treatment foster care parents within the Treatment Foster Care Oregon model in the Children’s Treatment Family Care program delivered in Kalamazoo County. Second, the position is responsible for providing case management services to families of a youth diagnosed with a serious emotional disturbance (SED) served in the FACT intensive home-based treatment program in Kalamazoo County. Case management includes providing interventions that focus on stabilizing the family; linking and/or referring to appropriate community services and support providers; conducting and coordinating necessary assessments; and developing and maintaining the treatment plan. The position hours include some evenings and weekends based on family needs.
Requirements
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Attend recruitment events which occur in the evenings and weekends.
· Recruit, training and retain treatment foster parents within the TFCO program.
· Provide interventions that focus on stabilizing the family; teaching and modeling effective strategies and coping skills; decreasing isolation; identifying, increasing, and utilizing natural strengths, supports, and resources; preparing for crisis situations; providing increased support during times of relapse; improving overall functioning. With a social work or counseling license, provide therapeutic interventions.
· Conduct and coordinate necessary assessments; develop treatment plan and provide for treatment reviews.
· Serve as advocate, linking and/or referring to appropriate community services and support providers.
· Assist consumers/families in the identification of, as well as building and sustaining positive natural supports in their lives, facilitating family meetings and providing direct supportive services to the consumer/family.
· Provide accurate monitoring and documentation of all case related activities to fulfill program obligations and requirements.
· Maintains good working relationships with therapeutic parents and TFCO staff.
· Participating in marketing for therapeutic parents includes advertising, prepares and places recruitment ads.
· Works with TFCO Coordinator to anticipate therapeutic parent recruitment needs.
· Serves as a resource to therapeutic families, listens to needs and maintains motivation and treatment and participation of therapeutic parents.
· Conducts ongoing telephone interviews with therapeutic parents 5 days a week.
· Assist in the collection and coordination with TFCO Coordinator weekly.
· Participates in weekly therapeutic parent meetings.
· Attends weekly clinical meetings with TFCO staffing team.
· Attends intensive home-based team meetings
· Coordinates with intensive home-based therapists on needs of families in program.
· Provides on-call assistance when needed.
· Complete 24 hours annually of child specific training.
· Provides transportation assistance as necessary or supports individuals in arranging transportation for appointments.
· Other duties as assigned.
EDUCATION AND EXPERIENCE:
Minimum qualifications
· Bachelor’s degree in a mental health-related field
· One year of experience in the examination, evaluation, and treatment of minors and their families
· A minimum of 1 year of experience in community organizing and/or advocating on behalf of youth and families is required. Must meet Michigan Department of Health and Human Services requirements for a Child Mental Health Professional credential within 120 days of hire.
· Experience in teaching, training or instructional presentations also preferred
· Must have a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of individual served.
· Ability to engage and maintain positive relationships with the youth and their family members; as well as develop collaborative relationships with community stakeholders.
· Ability to lead others and work cooperatively with a team.
· Committed to the implementation and fidelity of the TFCO model. Be self-directed, willing to take initiative, organized and detail oriented.
· Effective interpersonal communication skills, including strong written and verbal skills
Desired qualifications
· A familiarity with therapeutic parenting, preferably in a therapeutic home setting.
· Master’s degree of MSW or MA with licensure as a LLMSW, LMSW, LLPC, or LPC.
