Licensed Practical Nurse, Transitional Care Management -Greater Chicago Area

Strive Health is transforming kidney care by coordinating high-touch, data-driven support for patients with chronic kidney disease and end-stage renal disease. We are seeking an experienced Licensed Practical Nurse to act as a primary point of contact for patients across the Greater Chicago Area, coordinating resources, performing outreach and follow-up, providing education and health coaching, and supporting in-person care as needed.

Skills

  • Active LPN licensure in state of practice

  • Ability to work with confidential information using discretion
  • Reliable transportation and valid driver’s license
  • Basic digital literacy and internet connectivity for remote work

Responsibilities

  • Perform outbound and inbound calls to patients to assess clinical needs, provide education, schedule follow-ups, and connect to resources.
  • Coordinate appointments and communications with primary care physicians, nephrologists, specialists, and other providers.
  • Monitor hospitalizations and support care transitions by gathering and sharing relevant patient information with the care team.
  • Provide health coaching on disease and symptom management and follow up to ensure care plans are executed.
  • Partner with the Nurse Practitioner and care team to close care gaps and ensure continuity across locations.
  • Perform in-person patient care including home visits, clinic visits, and office-based tasks; may require physical activities such as standing, walking, and occasional lifting.

Education

  • Active LPN (Licensed Practical Nurse) licensure in state of practice
  • Minimum 5+ years combined related education, experience, or certification
