Responsibilities
- Perform outbound and inbound calls to patients to assess clinical needs, provide education, schedule follow-ups, and connect to resources.
- Coordinate appointments and communications with primary care physicians, nephrologists, specialists, and other providers.
- Monitor hospitalizations and support care transitions by gathering and sharing relevant patient information with the care team.
- Provide health coaching on disease and symptom management and follow up to ensure care plans are executed.
- Partner with the Nurse Practitioner and care team to close care gaps and ensure continuity across locations.
- Perform in-person patient care including home visits, clinic visits, and office-based tasks; may require physical activities such as standing, walking, and occasional lifting.
Education
- Active LPN (Licensed Practical Nurse) licensure in state of practice
- Minimum 5+ years combined related education, experience, or certification
