Skills
- Current COTA certification and valid state licensure
- Experience with home health or in-home patient care (preferred)
- Strong verbal and written communication skills
- Proficient documentation and basic EMR use
- Ability to travel to patient homes (valid driver’s license and reliable transportation)
Responsibilities
- Implement occupational therapy treatment plans as directed by the supervising OT
- Assist patients with therapeutic exercises, activities, and adaptive equipment
- Educate patients and caregivers on home exercise programs and safe ADL techniques
- Monitor, document and report patient progress to the supervising OT
- Collaborate with interdisciplinary team members to coordinate patient care
- Maintain accurate and timely patient records and compliance documentation
Education
- Certified Occupational Therapist Assistant (COTA) certification
- Valid state licensure to practice as a COTA
- CPR certification
