Responsibilities
- Assess, diagnose, implement, and evaluate patient responses to actual or potential health conditions during home visits
- Develop, implement, and update individualized care plans and document patient progress
- Provide patient and family education, counseling, and case finding to promote overall health and safety
- Deliver direct skilled nursing interventions that support life and restore health
- Coordinate care and collaborate with interdisciplinary healthcare teams and report to Nursing Supervisor/DON
- Maintain accurate clinical documentation in the agency EMR and follow regulatory/state requirements
Education
- Graduate of an accredited school of nursing recognized by the National League for Nursing
- Current and active RN license in good standing for CA (or state of practice)
- Meets state licensing educational and experience requirements
