- Valid California Physical Therapist license
- Proficiency with WellSky/Kinnser EMR or similar documentation systems
- Strong patient assessment and treatment planning skills
- Ability to safely perform and teach gait, balance, and transfer training
- Reliable transportation and ability to travel to patient homes
Responsibilities
- Provide individualized in-home physical therapy sessions and develop tailored treatment plans
- Improve patient strength, balance, mobility, and fall-risk reduction through therapeutic interventions
- Educate patients and families on safe daily activities and use of assistive devices
- Document patient progress, treatment plans, and outcomes in the WellSky/Kinnser EMR
- Coordinate care and communicate effectively with physicians, nurses, and case managers
Education
- Degree in Physical Therapy (DPT or MPT) or equivalent
- Current California PT license
- AHA CPR certification
