Responsibilities
- Provide individualized in-home physical therapy evaluation and treatment tailored to each patient’s goals.
- Develop and implement treatment plans to improve strength, balance, mobility, and reduce fall risk.
- Educate patients and family members on safe movement, ADLs, and home exercise programs.
- Document patient care, progress, and outcomes in WellSky/Kinnser EMR accurately and timely.
- Coordinate care with physicians, case managers, and interdisciplinary team members.
- Manage patient caseload, schedule visits, and maintain reliable travel between homes.
Education
- Licensed Physical Therapist (California) required
- AHA CPR Certification required
- Doctor of Physical Therapy (DPT) or Master’s in PT preferred
