About the Role:
As a Care Coordinator, you will be responsible for managing day-to-day care staffing for clients in both our In-Home (Kansas) and HCY (Missouri) programs. You’ll work closely with families, caregivers, and clinical teams to ensure quality care, timely communication, and continuity of services.
What You'll Do:
- Coordinate caregiver schedules based on client needs and caregiver qualifications
- Build meaningful relationships with families and caregivers to ensure trust and care satisfaction
- Maintain accurate documentation and reports per program guidelines
- Communicate clearly and professionally across departments and with external partners
- Assist with onboarding and training new caregivers to ensure a high standard of care
- Respond to staffing needs quickly and creatively to ensure client support
- Represent Phoenix with professionalism and empathy at all times
What You Bring:
- A true passion for working with children with special needs and the elderly
- Experience in home care, case coordination, scheduling, or human services preferred
- High school diploma or equivalent required
- Ability to multitask, problem-solve, and adapt quickly in a fast-paced environment
- Strong communication and organizational skills
