Registered Nurse RN - Universal Home Health Case Manager

Otterbein Universal Home Health is a faith-based, not-for-profit leader in senior care seeking a compassionate Home Health RN Case Manager to support and grow its home health services. The RN will provide skilled nursing care, manage a primary caseload, ensure compliance with Medicare and Ohio home health regulations, and collaborate with patients, families, and the interdisciplinary team to deliver high-quality in-home care.

  • Skilled clinical assessment and care planning

  • Medication administration knowledge
  • Strong patient and caregiver education skills
  • Ability to coordinate interdisciplinary care
  • Familiarity with Medicare home health regulations and Ohio licensure requirements

Responsibilities

  • Manage a primary caseload of home health patients and provide skilled nursing care in the home setting
  • Perform patient assessments, develop and update individualized care plans, and implement physician-ordered treatments
  • Document clinical care, progress notes, and outcomes in accordance with agency policy and Medicare documentation standards
  • Coordinate care with physicians, therapists, aides, and family/caregivers to ensure continuity and quality of care
  • Ensure compliance with Home Health Medicare Conditions of Participation and Ohio licensure laws
  • Educate patients and caregivers on disease management, medications, wound care, and safety
  • Participate in quality improvement activities and case conferences as needed

Education

  • Graduate of an approved nursing program (Associate or Diploma required; Bachelor's preferred)
  • Valid Ohio Registered Nurse (RN) license
  • CPR certification (or ability to obtain)
