Responsibilities
- Manage a primary caseload of home health patients and provide skilled nursing care in the home setting
- Perform patient assessments, develop and update individualized care plans, and implement physician-ordered treatments
- Document clinical care, progress notes, and outcomes in accordance with agency policy and Medicare documentation standards
- Coordinate care with physicians, therapists, aides, and family/caregivers to ensure continuity and quality of care
- Ensure compliance with Home Health Medicare Conditions of Participation and Ohio licensure laws
- Educate patients and caregivers on disease management, medications, wound care, and safety
- Participate in quality improvement activities and case conferences as needed
Education
- Graduate of an approved nursing program (Associate or Diploma required; Bachelor's preferred)
- Valid Ohio Registered Nurse (RN) license
- CPR certification (or ability to obtain)
