Responsibilities
- Plan, implement, and evaluate a calendar of group and individual activities that promote socialization, relaxation, entertainment, and daily living skills.
- Conduct in-depth assessments for each resident to tailor individual activity plans and document findings.
- Lead monthly council meetings and mentor activity assistants in program delivery and direction.
- Maintain and replenish supplies for indoor and outdoor recreation and ensure availability of out-of-home activities and outings.
- Complete weekly visits to evaluate resident engagement and create individualized opportunities for newly admitted residents per policy.
- Prepare and submit all activity-related reports and MDS documentation timely and accurately.
- Provide psychosocial support to residents, families, and partners and coordinate with interdisciplinary teams.
Education
- Post-secondary education preferred (social work, counseling, geriatrics preferred)
- MEPAP-1 required or must obtain within designated time
- Licensed Therapeutic Recreation Specialist will be considered
- Valid Ohio driver’s license required
