Responsibilities
- Provide ongoing mentoring to elder assistants in diversion and recreational enrichment activities.
- Maintain familiarity with each elder assistant’s experience, work style, and personality.
- Develop psychosocial profiles for elders and understand family dynamics.
- Conduct in-depth assessments of elders and prepare elder-specific activity materials.
- Participate in elder care planning and ensure availability of out-of-home activities.
- Include families in care and activity planning for elders.
- Evaluate program and activity effectiveness regularly.
- Perform weekly visits to each home to assess elder engagement.
- Create individualized opportunities for newly admitted elders per home policy.
- Document activities and assessments according to organization policy.
- Conduct mini mental assessments upon admission and inventory elders’ belongings.
- Perform other duties as assigned.
Education
- Associate degree in Social Services or related field preferred.
- Post-secondary education preferred.
- OH Licensed Social Worker preferred.
