Quality of Life Coordinator - Social Worker

Otterbein is seeking a compassionate Quality of Life Coordinator - Social Worker to provide psychosocial support and enhance the quality of life for elders in a home environment. This role requires strong interpersonal skills and experience in elder care to support both residents and their families effectively.

  • Recognize elder autonomy and dignity.
  • Communicate care, concern, and respect for elders.
  • Empower elders in decision-making wherever applicable.
  • Make prompt, accurate decisions regarding elder care and emergencies.
  • Work and communicate effectively within a self-directed work team.

Responsibilities

  • Provide ongoing mentoring to elder assistants in diversion and recreational enrichment activities.
  • Maintain familiarity with each elder assistant’s experience, work style, and personality.
  • Develop psychosocial profiles for elders and understand family dynamics.
  • Conduct in-depth assessments of elders and prepare elder-specific activity materials.
  • Participate in elder care planning and ensure availability of out-of-home activities.
  • Include families in care and activity planning for elders.
  • Evaluate program and activity effectiveness regularly.
  • Perform weekly visits to each home to assess elder engagement.
  • Create individualized opportunities for newly admitted elders per home policy.
  • Document activities and assessments according to organization policy.
  • Conduct mini mental assessments upon admission and inventory elders’ belongings.
  • Perform other duties as assigned.

Education

  • Associate degree in Social Services or related field preferred.
  • Post-secondary education preferred.
  • OH Licensed Social Worker preferred.
