We are hiring multiple Case Managers (Human Services Case Managers) to join our team. In this position, you will assess Oregonians applying for, or currently receiving, long-term care services to develop, coordinate, and arrange a variety of services to allow older adults and people with disabilities to get the continuity of care they need. Summary of Duties
As a Case Manager, you will:
Contact Oregonians or collateral contacts monthly to set up and monitor service plan and safety needs. This could include in person, the phone, or electronic contact.
Work closely and collaboratively with the Oregonian, other agencies, medical and care providers, and community partners to provide benefits, services, and information.
Work with Oregonians to create person-centered risk mitigation plans and provide risk mitigation as needed.
Interview Oregonians to determine initial and ongoing eligibility for healthcare, social services, and protective service needs. This can include collateral interviews both in person and on the phone. Develop detailed, individualized, and person-centered case planning for Oregonians.
Inform Oregonians of rights, responsibilities, service options, and methods of recourse.
Utilize all applicable forms, computer systems, tools, and any other available resources.
Minimum Qualifications
A valid driver’s license and acceptable driving record are required for this position.
A bachelor's degree in a Behavioral Science, Social Science, or a closely related field
OR
A bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)
OR
An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)
OR
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
Essential Attributes
We are looking for candidates with:
Experience determining eligibility and appropriate levels of service.
Experience interpreting and implementing policies and procedures.
Experience interviewing to obtain information to evaluate needs and next steps in the intake process.
Proficient in using computer software such: Microsoft Office, Word, Excel, Outlook, web browsing and communication applications.
Collecting, analyzing, and evaluating data while navigating databases for information.
Preference will be given to candidates that are bilingual in Spanish/English and Vietnamese/English. Please indicate on your application material if you have bilingual language skills.

General Information
This is a permanent, full-time position and is represented by the Service Employees International Union (SEIU).

Salary Information: Public Employee Retirement System (PERS) members in the executive branch (except AEE and AFSCME Parole & Probation) will get a 6.95% base salary increase and will pay a 6% employee contribution to PERS. Employees who are not yet PERS members (AA) will also get the 6.95% pay increase and start the 6% contribution when they join PERS (AP).
The step you start at will depend on the Equal Pay Analysis for the Human Services Case Manager:
AA: $4,409 - $6,420
AP: $4,715 - $6,866
