Position Overview:
We are seeking a highly motivated and experienced Registered Nurse to join our team as a Patient Care Manager in the Home Health sector, based in Roswell. This role is key in overseeing and ensuring the delivery of high-quality, compassionate care to patients within their homes.
Key Responsibilities:
- Lead and supervise a dedicated team of healthcare professionals to deliver exceptional patient care.
- Coordinate patient care services and develop individualized care plans.
- Conduct regular patient assessments and monitor the effectiveness of care plans.
- Ensure compliance with all healthcare regulations and standards.
- Collaborate with multidisciplinary teams to optimize patient outcomes.

Qualifications:
- Education: BSN degree preferred or equivalent clinical experience
- Licensure: Current Registered Nurse (RN) license in the state of Georgia
- Experience:
- Experience in home health care or a related field.
- Previous experience in a home care setting with two years of management or supervisory experience, preferred.
- Technical Skills: Proficiency in electronic medical records and healthcare technologies; knowledge of home health regulations and standards
- Soft Skills: Excellent communication and interpersonal abilities; strong organizational and problem-solving skills; leadership and team management capabilities
