Responsibilities
- Manage clinical care and rounding of patients at assigned skilled nursing facilities daily.
- Prescribe medications and treatments for residents at skilled nursing facilities.
- Provide high-quality, patient-centered comprehensive primary care services.
- Complete timely documentation of all patient encounters using Theoria’s EHR system ChartEasy™.
- Provide phone/secure messaging consultations with facility nursing staff as needed.
- Ensure punctual on-site presence for scheduled in-person shifts.
Education
- Completion of an accredited Physician Assistant program.
- Current certification and license to practice as a Physician Assistant.
