Responsibilities
- Manage clinical care of patients at assigned skilled nursing facilities (SNFs).
- Conduct daily rounding and prescribe treatments and medications to SNF residents.
- Provide high-quality patient-centered primary care in SNFs.
- Complete timely documentation of patient care in the proprietary EHR system ChartEasy™.
- Consult with facility nursing staff via phone or secure messaging as needed.
- Be onsite and punctual for scheduled in-person shifts.
Education
- Valid Nurse Practitioner licensure.
- Completion of an accredited Nurse Practitioner program.
