Responsibilities
- Oversee day-to-day operations of the CCM program and ensure high-quality patient care
- Supervise, mentor, and evaluate clinical staff including nurses, pharmacists, and other providers
- Develop and implement program strategies to improve CCM delivery and outcomes
- Coordinate care with interdisciplinary teams and external healthcare partners
- Participate in health IT and tech development projects to streamline CCM workflows
- Ensure regulatory compliance and maintain current knowledge of CCM guidelines
- Analyze patient and program data to identify opportunities for improvement
- Manage scheduling, licensure reimbursements, and staff development activities
Education
- Bachelor's degree in Healthcare Administration, Public Health, Nursing, or related field (required)
- Master's degree in a related field (preferred)
- Relevant clinical licensure or credentials as applicable
