Responsibilities

The Community Health Coordinator, under the direction of Business Development, will have dual responsibilities: 1. Lung Health/Nodule Coordination: The primary task for lung health/nodule coordination to decrease fragmented care, coordinate services and guide patient’s services through the healthcare system to receive efficient and effective diagnostic and medical treatment services, as well as facilitate their transition back to their primary care physician and community. The coordinator will also provide patient education to assist informed decision making, improve timeliness of care and coordinate multi-disciplinary care processes and supportive services. Additionally, the coordinator will be responsible for maintaining a central data base/navigation software system to assist with navigation, continuous quality improvement and clinical research. 2. EMS Coordination: The primary task for EMS coordination will be to serve as a liaison and advocate for private, local, regional, and state EMS agencies. In addition, this position will be responsible for ensuring collaboration across all care settings within the hospital as it relates to EMS and Public Safety relations.

Qualifications

Education and Work Experience

 

1. Valid State Respiratory Care Practitioner (RCP), Licensed Practical Nurse or Registered Nurse License required2. Current BCLS (AHA) certificate upon hire and maintain current.3. Minimum of 5 years of acute care or pulmonary experience preferred.4. Behavioral Violence Prevention (BVP) within 6 months of hire and maintain current annually.5. ACLS certification upon hire and maintain current
