Outsourcing Advantage is actively seeking a motivated and detail-oriented Intake Agent for Home Health Aide (HHA) Provider. In this vital role, you will be responsible for facilitating the intake process for individuals seeking home health aide services. You will serve as the first point of contact for potential clients and their families, ensuring a smooth and efficient entry into our care programs.
Responsibilities
- Process incoming referrals for home care services. Working promptly to respond to referrals thereby ensuring provider maximies all referral opportunities.
- Conduct initial assessments and gather necessary information from clients regarding their healthcare needs.
- Provide detailed information about available services, including care options and scheduling.
- Coordinate intake meetings, evaluations, on pending intakes to ensure timely completion.
- Assist clients and families with completing necessary intake documentation and answering any inquiries they may have.
- Maintain accurate and confidential client records in accordance with regulatory standards.
- Collaborate with internal teams to ensure effective communication and service delivery.
- Identify and address any issues or concerns during the intake process to ensure client satisfaction.
- Staff all new referrals with suitable aides
- Marketing/outreach to vendor/insurances to get more referrals
- Excellent verbal and written communication skills in English, with proficiency in additional languages being a plus.
- Strong organizational and multitasking abilities, with meticulous attention to detail.
- Previous experience in home healthcare, intake coordination, or a related customer service role.
- Proficient in using computer systems for data entry and client management.
- Ability to handle confidential information with integrity and professionalism.
- Compassionate and empathetic demeanor towards clients and their families.
- Ability to work both independently and as part of a collaborative team.
- Familiarity with HHA exchange software a plus
Hours
Full-time: Monday – Thursday 9-5 and Friday 9-12:30 EST
