Provider Success Consultant

Homeward is rebuilding healthcare delivery for rural America by partnering with health plans, providers, and communities to improve outcomes and access. They are seeking a Provider Success Consultant to build and maintain relationships with physicians and clinics across Michigan, drive lead generation and implementation, and support providers in succeeding under value-based care models.

Skills

  • Provider relations and clinical outreach experience
  • Lead generation and business development skills
  • CRM proficiency (Salesforce experience a plus)
  • Ability to interpret clinical and performance analytics
  • Strong consultative communication and relationship-building
  • Autonomy and initiative for field-based work
  • Willingness to travel regularly to rural Michigan

Responsibilities

  • Engage providers and introduce Homeward’s solutions to generate and qualify leads
  • Act as a consultant to identify provider needs and position Homeward as a partner
  • Document leads and manage a healthy pipeline in CRM
  • Support early implementation and coordinate cross-functional execution
  • Conduct regular in-person visits and provide ongoing relationship management
  • Interpret and explain provider performance dashboards and quality measures
  • Assess provider capabilities and create improvement plans to enhance reach and impact
  • Capture provider feedback for internal product, strategy, and operational improvements
  • Collaborate with clinical, analytics, and operations teams to deploy resources

Education

  • Bachelor’s degree required
  • MPH, MHA, RN, or equivalent experience preferred
