Patient Coordinator

Cadence Health is a technology-enabled remote care company focused on delivering proactive care to seniors to improve outcomes and reduce costs. The Patient Coordinator will support Cadence RNs in the Advanced Primary Care Management (APCM) program by coordinating appointments, managing patient communications, assisting with care coaching and social determinants of health, and maintaining accurate documentation to ensure patients stay connected to care.

Skills

  • Active CMA certification from AAMA, AMT, NHA, or NCCT
  • Comfort using digital platforms, EMRs, and patient care systems
  • Strong verbal and written communication skills
  • Organizational skills with ability to manage tasks and meet deadlines
  • Ability to work independently and escalate sensitive issues appropriately
  • Trustworthiness and experience handling sensitive patient information

Responsibilities

  • Call patients with care updates, reminders, and follow-ups on vaccines, diet, activity, and care plan check-ins
  • Send care goal emails and timely patient communications
  • Conduct proactive care coaching and perform 60-day engagement check-ins
  • Coordinate appointments with clinics and advocate for patient access to timely care
  • Provide courteous administrative support to patients via phone, email, and chat
  • Identify and connect patients to resources addressing social determinants of health (e.g., SNAP, transportation, food access)
  • Document all interactions accurately in Cadence systems in compliance with HIPAA and company protocols
  • Educate patients on Cadence technology and devices to maximize engagement
  • Escalate complex patient concerns to appropriate care teams for resolution

Education

  • Active Medical Assistant (CMA) certification (AAMA, AMT, NHA, or NCCT)
  • Additional healthcare-related education or training preferred but not required
