The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE
Under general direction provided by a licensed Social Worker, serves at the entry, trainee-professional level. Works on specific social work goals for an assigned patient unit to facilitate federal and state discharge planning requirements and/or adjustment of patient/families. Collaborates with hospital staff and mobilizes community resources relative to the adjustment and/or discharge planning in continuing care of individual patients.

ESSENTIAL FUNCTIONS

  • Under general direction provided by a Social Worker or Senior Social Worker, serves at the entry, trainee-professional level.
  • Works on specific social work goals for an assigned patient unit to facilitate federal and state discharge planning requirements and/or adjustment of patient/families.
  • As a member of a multidiscipline team, carries out the social work activity essential to patient care.
  • Establishes a casework relationship with patient family. Interviews patients/family members to obtain psycho-social history, discuss treatment plans, patient's adjustment and assist coping with home, health system and community problems or dynamics influencing patient's health.
  • Reviews clinical and psycho-social information for patients/families referred or those receiving care within assigned units with other professional staff.
  • Collaborates with other staff in developing and carrying out the health care plan for patients and families.
  • Seeks further clarification of services required.
  • Develops a plan for providing required services to meet psycho-social needs, including new approaches in situations without existing standard provisions for needs.
  • Provides casework counseling and direct services to individuals and families.
  • Records all patient care activity in medical chart according to established policies.
  • Evaluates patient/family psycho-social status relative to discharge planning and/or adjustment needs.
  • Recommends and carries out discharge plans or other plans for continued ancillary care.
  • Interprets patient/family needs and service recommendations to hospital staff.
  • Consults with and reviews assignments regularly with supervisor
  • Refers more difficult assignments to senior staff.
  • Represents the division in assigned health team rounds.
  • Develops a thorough knowledge of hospital and community resources.
  • Corresponds with other institutions and community agencies.
  • Meets on a mutual orientation basis with other agencies to review program information, procedures, techniques, etc.
  • Acts as the hospital and patient/family liaison with other community services.
  • For specific patients, interprets hospital services and facilitates psycho-social plans with other agencies.
  • Maintains cooperative working relationship with community agencies and other resources.
  • Reviews and discusses social work and other health related literature with supervisor to expand knowledge of social work techniques, procedures, and innovations.
  • Participates in workshops and symposia and shares in formulating policies and procedures for the division.
  • Serves on various division committees.
  • Participates in in-service training, departmental, hospital and special meetings to remain current on clinical, organizational and community developments.

MINIMUM EDUCATION & EXPERIENCE

  • Bachelor's degree in Social Work required
  • Or equivalent combination of education and experience
  • Clinical or Community Social Work experience in health setting preferred
