- Current LPN license in North Carolina or a compact state
- Current in-person CPR certification
- TB skin test completed within the past 12 months
- Experience caring for trach- and ventilator-dependent patients
- Strong clinical assessment and medication administration skills
- Ability to work independently in a home setting and manage case-specific assignments
- Clear communication and documentation skills
Responsibilities
- Provide individualized, one-on-one nursing care in patients' homes for adult and all-age case-specific clients
- Manage tracheostomy and ventilator-dependent patient care, including ventilator setup, monitoring, and troubleshooting
- Administer medications, perform treatments, and monitor vital signs and respiratory status
- Collaborate with families, caregivers, and 24/7 Respiratory Therapist support to maintain continuity and safety of care
- Document patient status, interventions, and care plans accurately and timely in company records
- Maintain and operate home medical equipment according to protocol and infection-control standards
- Communicate clinical concerns promptly to supervisors, clinicians, and families
Education
- Graduation from an accredited LPN/Vocational Nursing program
- Current LPN licensure (NC or compact)
- Current CPR (in-person) certification
- Up-to-date TB screening within 12 months
