Responsibilities
- Conduct psychosocial assessments and develop individualized care plans with the IDT
- Deliver and document social work interventions including counseling, case management, and resource coordination
- Coordinate and facilitate care conferences with participants, families, and care team members
- Support care transitions between settings (hospital, SNF, home) and provide discharge planning
- Act as liaison between participants, families, caregivers and clinical team to promote continuity of care
Education
- Master’s Degree in Social Work (MSW) required
