Responsibilities

- Monitor, document, and report patient progress and changes to the supervising OT
- Coordinate with other healthcare professionals to support quality, patient‑centered care
- Maintain accurate, timely patient records and comply with agency documentation standards
- Travel to patient homes; manage schedule and time effectively
Education
- Completion of an accredited Occupational Therapy Assistant (OTA) program
- Associate degree in Occupational Therapy Assistant or equivalent
- Current COTA certification
