Responsibilities
- Complete initial and ongoing patient and family assessments to determine home care needs
- Develop, initiate and revise individualized home health plans of care
- Use assessment data to establish nursing diagnoses and measurable goals
- Administer medications and treatments per physician orders
- Provide patient and family education and counseling related to nursing needs
- Coordinate care with physicians, community providers, and the interdisciplinary team
- Supervise, instruct and evaluate home health aide care; review and co-sign LPN visit records and orders
- Document clinical notes, update primary physician at required intervals, and ensure compliance with state and federal home health regulations
- Identify and implement discharge planning needs and arrange necessary equipment and services
- Participate in on-call duties as defined by policy
Education
- Graduate of an approved nursing program (ADN or BSN)
- Bachelor of Science in Nursing (BSN) preferred
- Valid Ohio Registered Nurse (RN) license required
- Current CPR certification required
- Licensed driver with automobile insurance meeting Ohio requirements
