Responsibilities
- Direct and coordinate resident care, including assessment, medication administration, and basic treatments
- Collaborate with physicians, nurses, therapists, residents and families to develop and follow care plans
- Maintain accurate clinical documentation and electronic health records
- Provide emotional support and dignity-centered care to residents and families
- Promote resident autonomy and involve residents in decision-making
- Monitor and report changes in resident condition and escalate per policy
- Ensure a safe, clean environment and follow infection control procedures
- Work assigned shifts (7a-7p, 7p-7a, 11p-7a) including full-time, part-time and PRN coverage as needed
Education
- Graduate of an approved nursing program (LPN)
- Valid Ohio Licensed Practical Nurse (LPN) license
- Current CPR certification
