Responsibilities
- Complete initial and ongoing patient and family assessments to determine home care needs
- Initiate, implement, and revise individualized plans of care and nursing diagnoses
- Administer medications and treatments as prescribed and document clinical notes
- Provide patient and family education, counseling, and discharge planning
- Communicate with physicians and community health partners; obtain and follow physician orders
- Coordinate equipment and services needed for home care and supervise home health aides
- Participate in on-call duties per agency policy and ensure timely physician updates
Education
- Graduate of an approved nursing program required
- Bachelor of Science in Nursing (BSN) preferred but not required
- Current Ohio Registered Nurse (RN) license and CPR certification required
