Responsibilities
- Coordinate development, completion, and transmission of MDS assessments in compliance with regulatory requirements
- Manage and review reimbursement and Medicare-related documentation
- Case manage individualized resident care plans, including rehabilitative programs
- Meet with residents and families to discuss care goals and plan updates
- Collaborate with interdisciplinary team members (nursing, therapy, social work, pharmacy) to ensure coordinated care
- Maintain accurate, timely clinical documentation and support audit readiness
- Participate in on-call rotation and respond to care-related emergencies as needed
- Provide leadership in clinical standards, regulatory compliance, and quality improvement initiatives
Education
- Current Ohio RN license required
- BSN preferred but ADN/diploma candidates will be considered
- MDS experience in long-term care preferred; experienced RNs without MDS experience will be trained
