Responsibilities
- Coordinate development, completion, and transmission of MDS assessments per regulatory requirements
- Manage and review reimbursement and MDS-related documentation
- Develop and case manage individualized resident care plans, including rehabilitative programs
- Meet with families to discuss resident care and care planning
- Ensure compliance with long-term care and Medicare regulations and clinical standards
- Participate in the clinical on-call rotation, including every 4th weekend
- Collaborate with interdisciplinary team members to deliver high-quality, person-centered care
- Make prompt and accurate clinical judgments regarding resident care and emergencies
Education
- Current Ohio RN license preferred (BSN preferred)
- LPN with prior MDS experience may be considered
- Relevant continuing education or MDS training preferred
