MDS Nurse

- Proficiency with MDS assessments and care planning

Responsibilities
- Coordinate development, completion, and transmission of MDS assessments per regulatory requirements
- Manage reimbursement review and submission processes
- Case manage care plans, including rehabilitative programs
- Meet with residents and families to discuss care planning and resident needs
- Ensure clinical documentation meets standards and regulatory compliance
- Participate in clinical on-call rotation (every 4th weekend)
- Lead and deliver person-centered, high-quality clinical services
Education
- Current Ohio Registered Nurse (RN) license
- BSN preferred
