Responsibilities
- Provide direct nursing care and supervision to residents, including medication administration and basic treatments
- Assess resident condition, monitor vitals, and report changes to RN/physician
- Document care, assessments, and interventions accurately in the medical record
- Collaborate with interdisciplinary team to develop and implement individualized care plans
- Support residents' dignity, autonomy, and psychosocial needs
- Communicate with residents' families and the care team about resident status and care goals
- Adhere to facility policies, state regulations, and infection control protocols
- Work assigned shifts including potential 2nd/3rd shift differentials, weekend and holiday rotations as scheduled
Education
- Graduate of an approved practical nursing program
- Current Ohio Licensed Practical Nurse (LPN) license
- CPR / BLS certification
