JF&CS is now seeking a Full-Time Program Coordinator, Memory Support Services.


PURPOSE: 

The Memory Support Services Program Coordinator is responsible for the organization, delivery, and tracking of programs that support older adults living with memory loss or dementia and their family caregivers. These services include: 

  • Brain Health Bootcamp - Weekly small-group classes for older adults in the early to moderate stages of cognitive decline, focused on brain and body exercises, memory strategies, and social connection.
  • Music & Memory-A personalized music program that uses MP3 players or Simpl devices to reduce agitation, anxiety, and isolation for individuals with moderate to late-stage dementia.
  • Therapeutic Pets - Robotic pets provided to homebound clients with dementia and to children and adults receiving JF&CS clinical services, used to reduce stress and enhance well-being.
  • Memory Lane TV-A streaming platform offering therapeutic video programming to improve engagement, quality of life, and behavior for individuals with cognitive decline.
  • One Good Deed Dementia Respite Program - A volunteer-based respite program that matches trained community members with individuals living with dementia to provide social engagement and relief to caregivers.

This role ensures these programs are implemented with excellence and compassion, while maintaining strong relationships with caregivers, contractors, volunteers, and internal staff. 


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following (other duties may be assigned): 


Program Coordination & Implementation 

  • Oversee the day-to-day operations for all Memory Support Services programs listed above.
  • Assist supervisor with coordinating Brain Health Bootcamp, including participant intake, Zoom/transportation assistance, attendance tracking, contractor support, room setup, and payment processing.
  • Oversee operations of Music & Memory and Memory Lane TV, including:
  1. Working with caregivers and volunteers to select personalized music or video content.
  2. Downloading content to MP3 players, Simpl devices, or streaming platforms.
  3. Providing clear written and verbal instructions for device use.
  • Mailing or delivering devices and offering additional support via Zoom, FaceTime, or in person.
  • Distribute and manage robotic Therapeutic Pets for homebound individuals with dementia and clinical clients who would benefit from emotional support.

One Good Deed Dementia Respite Program 

  • Collaborate with One Good Deed staff to coordinate volunteer-client matches that offer meaningful companionship to individuals living with dementia and much-needed respite for their caregivers.
  • Plan and facilitate volunteer training sessions and planning meetings in partnership with the Contract Memory Support Services Specialist and other Aviv team members.
  • Organize and support quarterly in-person programming for matched volunteer-client pairs to promote social engagement.
  • Track and analyze program data to assess effectiveness and identify opportunities for improvement.
  • Assist with documentation and data collection to support the development of a replicable training and program toolkit.

Marketing & Outreach 

  • Partner with marketing team to market Memory Support Services to older adults and caregivers through flyers, newsletter articles, and referrals.
  • Respond to inquiries generated from outreach and promotional materials.
  • Collaborate with JF&CS staff to identify and enroll eligible clients and caregivers.

Supplies, Equipment & Technology 

  • Order and manage inventory of therapeutic pets, MP3 players, Alexa devices, headphones, and related program supplies.
  • Coordinate Internet service access for Alexa device users.
  • Track delivery, functionality, and replacement of all devices and therapeutic tools.

Evaluation, Reporting & Administration 

  • Work with the Aviv Administrative Assistant to send, collect, and summarize participant surveys for continuous improvement.
  • Maintain detailed records of participant engagement, feedback, and program outcomes.
  • Process program invoices and payments in Salesforce.
  • Draft and submit quarterly programmatic and financial reports to funders (e.g., NJHSA for Memory Lane TV).
  • Attend regular supervision and team meetings.

Other Duties 

  • Carry out additional tasks assigned to support the growth and success of Memory Support Services.

Requirements

The ideal candidate will demonstrate: 

  • A client-centered mindset and a strong commitment to improving the quality of life for older adults and their family caregivers.
  • Experience coordinating programs or services for older adults, especially those with dementia or cognitive impairments.
  • Strong organizational skills, with the ability to manage multiple programs simultaneously, coordinate logistics, and track program outcomes.
  • Comfort using technology to support virtual programs (e.g., Zoom, streaming platforms, MP3 players, Alexa), and the ability to develop simple user guides for clients and caregivers.
  • Familiarity with dementia-related services, memory support interventions, and the value of volunteer-led engagement programs.
  • Excellent communication skills-written and verbal-and ability to effectively collaborate with caregivers, volunteers, contractors, and internal staff.
  • Detail-oriented with the ability to track participation, support program evaluation, and manage supply ordering, invoicing, and reporting.
  • Strong interpersonal skills, including empathy, patience, and professionalism when interacting with older adults, caregivers, and volunteers.
  • Flexibility and initiative to adapt to evolving program needs.
  • Proficiency with Microsoft Office Suite and comfort using client databases or CRM systems (e.g., Salesforce).

EDUCATION and/or EXPERIENCE: 

Master’s degree in social work or other area of human service preferred, with at least 2 years related experience working with elders and family caregivers, as well as strong knowledge of community resources. Knowledge of local resources, a plus. 

 


Candidates who are selected for employment must successfully complete a pre-employment background screening as well as a pre-employment drug screening. Employment will be contingent upon the satisfactory completion of both the background check and the drug screening. Additionally, if applicable, tuberculosis testing (TB) and fingerprinting may also be required.
