Clinical Resource Nurse

We’re looking for a skilled and compassionate Resource Nurse (RN) to join our team in Las Vegas. This role is ideal for a nurse who values clinical excellence, works well in a fast-paced environment, and is committed to supporting care teams in delivering high-quality, home-based care.

Key Responsibilities
  • Conduct nursing comprehensive assessments and re-evaluations
  • Perform detailed care plan reviews and assist with updates
  • Educate patients, caregivers, and HHAs on disease progression, symptom management, and safe interventions
  • Ensure accurate, timely, and compliant documentation aligned with DEEOIC and agency standards
  • Identify and monitor for trends such as hospitalizations, falls, and care plan non-adherence
  • Serve as a clinical resource for care team members and field staff navigating patient care concerns or decision points.
  • Participate in case conferences, interdisciplinary planning, and coordination of care
  • Review clinical documentation for early signs of patient decline or emergent needs and advocate for appropriate adjustments and services
  • Coordinate with physicians and care teams to ensure safe, timely, and effective interventions
  • Assist with performance improvement initiatives and support QAPI efforts as needed
  • Maintain flexibility in responding to urgent patient needs and unexpected care coordination issues.


Qualifications – Required
  • Active and unrestricted RN license in the State of Nevada.
  • Strong working knowledge of home health care planning, nursing interventions, and staff scope of practice.
  • Ability to work independently, manage multiple priorities, and travel to patient homes as needed.
  • Proficient clinical documentation and understanding of home health regulatory standards.


Preferred Qualifications
  • Experience working with diverse patient populations.
  • Strong organizational, time management, and communication skills.
  • Ability to adapt to varying patient needs, field environments, and schedule changes.


Compensation & Schedule
  • Compensation: $45/hour
  • Flexible scheduling based on operational needs and patient acuity


$45 - $45 an hour
