- Current RN license (Compact license preferred)

- Strong knowledge of HIPAA and working from a secure remote environment
- Ability to work Eastern Time Zone business hours (8:30a - 5:00p EST)
Responsibilities
- Manage an assigned patient panel and create individualized, actionable care plans
- Establish trusting relationships with patients and caregivers to provide clinical and emotional support
- Assess patients, identify needs including social determinants of health, and translate into care interventions
- Coordinate interdisciplinary team members, physicians, and community resources to ensure continuity of care
- Monitor and adjust care plans, track outcomes, and support transitions across care settings
- Follow evidence-based care management guidelines and workflows to meet quality and regulatory requirements
- Participate in clinical huddles and patient care conferences and prepare reports on program metrics
- Identify cost-effective interventions that align with value-based care goals
- Engage in continuous process improvement and patient/caregiver education
Education
- Associate degree in nursing (required)
- Current RN license required
