Case Management Coordinator

Abby Care is a mission-driven, VC-backed healthtech startup building a family-first care platform to support paid family caregiving. We’re seeking a detail-oriented Case Management Coordinator in Denver to manage Colorado waiver prior authorizations, develop strong CMA relationships, drive reauthorizations and appeals, and ensure uninterrupted long-term home health (LTHH) services across Colorado waiver programs.

Skills

  • Deep working knowledge of Colorado HCPF policies, PAR processes, and ColoradoPAR system
  • Proven experience coordinating with county case managers and CMAs in Medicaid LTHH settings
  • Strong stakeholder management and relationship-building skills
  • Ability to manage multiple concurrent cases with attention to documentation and compliance
  • Experience building processes, delivering trainings, and tracking performance metrics

Responsibilities

  • Act as primary liaison with county and independent case managers for PARs, authorizations, reauthorizations, and appeals.
  • Build and maintain relationships with Colorado Case Management Agencies; lead regular meetings and joint case reviews.
  • Co-develop integrated care plans with case managers, families, and clinical teams aligned to Medicaid waiver requirements.
  • Manage Colorado-specific authorization operations for HCBS waiver programs (CHCBS, EBD, CES, CFC, SLS) ensuring HCPF compliance.
  • Design and drive the reauthorization process to prevent service lapses and meet sub-10-day processing targets.
  • Provide expert guidance on complex cases (multiple waivers, emergency authorizations, appeals) to internal teams and CMAs.
  • Create and deliver training on Colorado PAR requirements and LTHH waiver nuances; mentor team members and set performance metrics.

Education

  • Bachelor’s degree in Healthcare Administration, Business, or related field preferred
  • Equivalent combination of relevant experience accepted
  • Colorado Medicaid certification and LTHH authorization training strongly preferred
