Shared Living Coordinator

Under the direction of Vista Care’s Shared Living Area Director, the Shared Living Coordinator is responsible for reviewing and responding to referrals for services in their area. This position will work closely with Case Management Agencies, families, persons seeking services, and Host Home Providers to ensure a positive match is made for placement. Work is typically performed with the people supported in an administrative office, remotely, in Shared Living settings, and in the community

  • Responsible for developing a referral pipeline for persons supported, Shared Living Providers, Family Caregivers, and Respite Providers.
  • Recruitment and interviewing of potential Shared Living Providers, completion of background checks, organizing drug screens as applicable, and processing applications. 
  • Collection, coordination, and recording of a database of referral responses, and successful, attempted, and unsuccessful placements. 
  • Participates in the Shared Living Provider/Person Supported matching process via meet & greets, coordinates visits between the Providers and Persons Supported in compliance with local, state, and federal guidelines and/or Vista Care’s best practices. 
  • Maintains contact information for current and potential Shared Living Providers, Family Caregivers, and Respite Providers. 
  • When responding positively to referrals; meets with the person seeking services to complete assessments and determine their needs, preferences, goals and desires. Uses this information to help create a potential match with existing or potential Providers. 
  • Provides initial training and support to the Shared Living Providers to ensure compliance with local, state, and federal regulations and Vista Care’s best practices. 
  • Creates the initial master record for uploading into the Electronic Health Record.
  • May be required to provide initial supervisory duties for new Shared Living Providers for up to 90 days and/or may be required to carry a more permanent caseload. 
  • Assists Shared Living Providers with identifying potential Respite Providers and helps to coordinate Respite care when needed.
  • May be required to respond to emergency referrals from external sources such as hospitals, nursing homes, or other settings, and to assist in rapid placement of the person in a Shared Living setting if there is a good match between the potential Provider, the Person Supported, and Vista Care. 
  • Must carefully analyze potential placements for risks to the Provider, the Person Supported, the community, and Vista Care as an organization. 
  • Will be part of a placement review team to include local management, upper management, nursing, quality and/or other departments to ensure the placement will be successful, safe, and secure for all involved. 
  • Participate in community events and sponsorships.
  • Build relationships with local external stakeholders including but not limited to churches, referral sources, schools, case management agencies, advocacy agencies and parent/guardian groups.
  • Complete home inspections within Housing Quality Standards (HQS) and/or Department of Housing and Urban Development (HUD) guidelines and/or Vista Care’s best practices in Shared Living settings.
  • In-state travel required, up to 75% of the time.
  • Other duties as assigned.

  • Must be at least 18 years of age
  • Must successfully complete all certification requirements within the assigned state(s) of operation. 
  • Valid driver’s license and clear driving record in compliance with local, state, and federal regulations and Vista Care’s best practices
  • Three (3) or more years experience in human services or a related field preferred
  • Experience in shared living for people with intellectual or developmental disabilities preferred
  • Clear background check in compliance with local, state, and federal regulations and Vista Care’s best practices
  • High School Diploma or GED Required
  • A college, or university degree or the equivalent is preferred. Social services or similar fields preferred.
  • Physical requirements 
    • Requires repetitive manual and finger dexterity and eye hand coordination
    • Ability to use all office equipment; 
    • Ability to lift/carry up to 50 pounds using appropriate body mechanics. 
    • Must be able to remain awake and alert for the duration of each shift, bend, stoop, push, pull, reach, twist, sit, and walk for periods of time. 
    • Must be able to utilize proper body mechanics while assisting with the transfer of individuals weighing up to 250 pounds utilizing manual or mechanical lifting devices and/or two-person transfers. 
  • Visual, hearing, and communication requirements:
    • Requires corrected vision and hearing to normal range, with or without reasonable accommodation. 
    • Must be able to communicate verbally and in writing with coworkers, Independent Contractors, other professionals, and people supported. 
  • Work location:
      • Primarily office settings or remote with some of the time spent in Shared Living settings or the community with varying degrees of background noise or surrounding noise levels. 
      • The possibility of exposure to blood borne pathogens exists if universal precautions are not followed. 
      • May require working under stressful conditions at times and handling emergencies. There will be pressure at times to meet scheduled appointments and deadlines and manage crisis situations. 
      • This position will interact with individuals who are intellectually and/or developmentally disabled and may be frail or confused and have the potential for verbal or physical aggression
      • Variable weekly hours to support operational needs including evenings, nights, weekends, and holidays including being responsible to be on-call for assigned program(s).
