Pay: Regular visit $45–$50 • SOC $100–$120 • ROC $90–$100 • Discharge $50–$60
Employment Type: PRN, Part-Time
Schedule: You choose days and coverage area; every effort made to group nearby visits

Job Description

Overview

Serve patients at home across Collin, Dallas, Denton, Tarrant, Parker, Wise and surrounding counties with a CHAP- and Medicare-certified agency committed to outcomes and patient experience.

Responsibilities

  • Lead OASIS SOC and ROC visits with accurate, timely documentation

  • Complete recertifications and safe discharges with thorough education

  • Communicate plan changes to providers and office staff

  • Ensure compliance with Medicare and agency standards using WellSky Kinnser

  • Champion safety, infection control, and patient advocacy in the home

Qualifications

Required

  • Unencumbered Texas RN license

  • At least 1 year recent home health experience including OASIS

  • WellSky Kinnser proficiency

  • Reliable vehicle and current driver’s license
    Preferred

  • BSN or higher

  • Prior hospice, wound care, or chronic disease education experience
