Responsibilities
- Develop and maintain personalized care plans addressing clinical and non-clinical patient needs
- Conduct regular assessments onsite and virtually to identify medical or social risks
- Perform proactive outreach to patients and clinics per program guidelines
- Manage a diverse patient panel and coordinate complex case management per NCQA standards
- Communicate care updates to care teams and families and provide tailored patient education
- Liaise with dialysis facilities to support care transitions and maintain accurate program records
- Use Strive data platforms to identify trends and areas of focus
- Provide culturally sensitive care and collaborate with clinical team members
- Document thoroughly in EMR and program software; perform in-person visits as needed
Education
- Associate's degree in Nursing (required)
- Current RN license in the state of practice (required)
- BLS/CPR certification (required)
- BSN preferred but not required
