Responsibilities
- Conduct comprehensive psychosocial assessments and provide supportive counseling tailored to patient needs.
- Screen for and address social determinants of health; locate and coordinate community resources and services.
- Use motivational interviewing and behavior-change techniques to develop patient-centered goals and care plans.
- Collaborate with interdisciplinary teams (primary care, nephrology, dialysis, transplant) to coordinate care.
- Educate and support patients and families on adjustment to chronic illness, advance care planning, and quality-of-life issues.
- Provide in-person care as needed, including home visits and clinic-based interactions.
Education
- Master's degree in Social Work (MSW) required.
- State-specific clinical social work licensure required (LCSW or equivalent).
- Proof of passing the Association of Social Work Boards (ASWB) exam required.
