Home Health & Hospice Social Worker

Otterbein is hiring a compassionate Home Health & Hospice Social Worker to provide comprehensive psychosocial services, counseling, and bereavement support to terminally ill patients and their families in the West Liberty, OH area. This PRN field position requires strong assessment, documentation, and care coordination skills to support the interdisciplinary team and connect patients/families with community resources.

Responsibilities

  • Conduct initial comprehensive psychosocial and bereavement risk assessments within five calendar days of hospice election.
  • Provide psychosocial and anticipatory grief counseling to patients, families, caregivers, and significant others.
  • Make and facilitate referrals to community resources, including financial assistance and placement services when appropriate.
  • Participate in development of the plan of care and interdisciplinary team (IDT) meetings to coordinate patient-centered care.
  • Prepare and maintain social histories, evaluations, care plans, and timely clinical documentation per agency guidelines.
  • Assist with discharge planning, respite, inpatient placement, and transitions of care as needed.
  • Report complaints, allegations of abuse, and compliance issues; ensure respect and dignity in all patient interactions.
  • Mentor and participate in orientation, field training, and in-service education for staff, contractors, and referral sources.

Education

  • Master's degree in Social Work from a CSWE-accredited school
  • Active Ohio Social Work license (state licensure required)
