Responsibilities
- Conduct initial comprehensive psychosocial and bereavement risk assessments within five calendar days of hospice election.
- Provide psychosocial and anticipatory grief counseling to patients, families, caregivers, and significant others.
- Make and facilitate referrals to community resources, including financial assistance and placement services when appropriate.
- Participate in development of the plan of care and interdisciplinary team (IDT) meetings to coordinate patient-centered care.
- Prepare and maintain social histories, evaluations, care plans, and timely clinical documentation per agency guidelines.
- Assist with discharge planning, respite, inpatient placement, and transitions of care as needed.
- Report complaints, allegations of abuse, and compliance issues; ensure respect and dignity in all patient interactions.
- Mentor and participate in orientation, field training, and in-service education for staff, contractors, and referral sources.
Education
- Master's degree in Social Work from a CSWE-accredited school
- Active Ohio Social Work license (state licensure required)
