Emergency Department Follow-Up (LPN Required)

Imagine Pediatrics is a pediatrician-led, tech-enabled medical group delivering 24/7 virtual-first and in-home care for children with special health care needs. They seek an LPN to provide telephonic and occasional in-person emergency department follow-up, care coordination, and discharge support to medically complex pediatric patients, working closely with families, hospitals, and interdisciplinary teams.

Skills

  • Licensed Practical Nurse (LPN) licensure required
  • Bilingual Spanish required (verbal and telephonic communication)
  • 2+ years clinical experience, preferably in emergency or inpatient settings
  • Experience with pediatric and medically complex populations preferred
  • Strong telehealth/telephonic care coordination skills
  • Familiarity with Medicaid population and community resource referral pathways
  • Proficient with EHRs and virtual communication platforms

Responsibilities

  • Proactively engage patients after ED visits and provide education on ED avoidance and available resources
  • Conduct virtual and telephonic follow-up visits and, when needed, coordinate in-person/community supports
  • Identify symptom worsening or clinical concerns during post-discharge contacts and escalate to clinical team
  • Coordinate pharmacy fills, PCP follow-up, SDOH referrals, and schedule necessary callbacks with clinical teams
  • Collaborate with facility discharge planners and Transition of Care Nurses to remove barriers to after-hospital care
  • Serve as liaison with hospitals and health systems to raise awareness of Imagine Pediatrics services
  • Attend daily huddles and work within a multiplatform virtual environment as part of an interdisciplinary team
  • Provide internal, social service, and community-based referrals and document encounters in EHR

Education

  • Associate's degree or higher
  • Active Licensed Practical Nurse (LPN) credential
