The Dialysis Social Worker is responsible for assessing and responding to the psychosocial needs of our dialysis patients to ensure the best possible outcomes and improve quality of life. We are seeking a compassionate and resourceful Renal Social Worker to support patients navigating the emotional, financial, and lifestyle challenges associated with chronic kidney disease (CKD). This role is essential in helping patients understand their treatment options, access community resources, and improve their overall quality of life through individualized care planning and advocacy.
Schedule: Fulltime Monday through Friday, weekends off
Compensation: $75K - $85K. Pay range depending on dialysis experience
