Responsibilities
- Run EPIC reports to collect and track program metrics under Palliative Care Manager guidance.
- Perform MATCH audits and outreach to MATCH patients.
- Coordinate and schedule clinician visits in line with contract obligations.
- Partner with contracted vendors, hospitals, and facilities to facilitate referrals and educate on programs and team approach.
Education
- Associate degree or equivalent from a two-year college or technical school.
- Equivalent combination of education and relevant experience.
