Responsibilities
- Provide one-on-one or 2:1 outpatient/home-visit physical therapy care based on assigned caseload
- Manage an individualized caseload (typical range 10–40 visits/week) and coordinate visit scheduling
- Complete timely, efficient documentation using company systems
- Use outcome data to inform treatment plans and drive patient progress
- Participate in structured onboarding and ongoing training to align with company care models
- Communicate effectively with patients, families, and interdisciplinary teams regarding care goals and progress
Education
- Doctor of Physical Therapy (DPT) or equivalent degree required by state
- Minimum 3 months of clinical experience as a licensed PT
