Responsibilities
- Provide personalized in-home physical therapy sessions and develop tailored treatment plans
- Improve patient strength, balance, mobility, and reduce fall risk through evidence-based interventions
- Educate patients and families on safe daily activities, assistive device use, and fall prevention
- Document treatment plans, progress, and outcomes in the WellSky/Kinnser EMR system
- Coordinate care with physicians, case managers, and interdisciplinary team members
Education
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) or equivalent
- Active California Physical Therapist license
- Current AHA CPR certification
