Key Responsibilities
- Deliver nursing care to residents, including assistance with activities of daily living (ADLs).
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor and document patients’ vital signs, health status, and responses to treatment.
- Collaborate with RNs and other healthcare team members to create and implement individualized care plans.
- Provide education and support to residents and their families regarding health issues and care processes.
- Maintain a safe, clean, and respectful environment for patients and staff.
- Comply with all facility policies and procedures while providing care in accordance with established standards.
- Valid LPN license in the state of New York.
- Graduation from an accredited practical nursing program.
- Experience in long-term care or similar healthcare settings preferred.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with a multidisciplinary team.
- Strong organizational and time-management skills.
- Basic Life Support (BLS) certification is required.
- Flexibility to work varying shifts, including evenings and weekends as needed.
