Job Description
Spectrum Healthcare Resources has a potential opportunity for a Registered Nurse (RN) - Case Manager at Tyndal Air Force Base near Panama City Florida.
RN Position Details: Ind.Nurse
Responsibilities:
- Coordinate and manage the care of patients through the healthcare continuum.
- Conduct assessments to identify patient needs and develop individualized care plans.
- Collaborate with healthcare providers to ensure timely and effective delivery of services.
- Monitor and evaluate patient progress, adjusting care plans as necessary.
- Provide patient education and support to enhance self-management.
- Advocate for patients to ensure they receive the highest quality of care.
Qualifications:
- Current Registered Nurse (RN) license in the state of South Dakota.
- Bachelor's degree in Nursing (BSN) preferred.
- Certification in Case Management (CCM) is a plus.
- Strong organizational and communication skills.
- Ability to work collaboratively in a team-oriented healthcare setting.
Experience:
- Minimum of 2 years of clinical nursing experience.
- Previous experience in case management or care coordination.
- Familiarity with electronic health records.
